INSTRUCTIONS: Alums and guests registered using this form are having their NAME TAGS printed in advance. Registered attendees can then SKIP flling out their own name tag in the regular check-in process. Enter one name, class year association, and the total number of attendees at ONE (1) for each name tag to PRINT. This will require the form to be used multiple times for multiple name tags. After submitting a registration, you can easily return to register another picnic attendee by using the button provided in the success form.
Alternatively, enter the total number of attendees if one name tag is printed and all other attendees will register separately and create their own name tag at the picnic. Thank you for registering using this form. It helps our picnic committee prepare for the annual picnic.
For this form, do not use ALL CAPS in any form field. Use a dash in between your maiden and married last names. I.E., "Maiden-Married" is in the last name field for female Vocational alums. Enter an emergency contact phone or email in the field below if you do not receive Vocational's Newsletter

EMERGENCY CONTACT METHOD: The picnic committee needs a record of everyone attending the annual picnic. In the case of some emergency, they need to contact people. If you receive our Vocational Newsletter, the picnic committee can use that email to contact you if needed. If you do not receive our newsletter, enter your emergency phone number or email address for contacting you if required. It's crucial to understand that failure to be on our newsletter email list or to enter an emergency contact method below means we cannot contact you if something happens at the picnic that requires contacting you. I appreciate your cooperation.
